Job Description

  • Provide trauma informed, intensive case management services to women, families, and youth who are experiencing chronic homelessness
  • Work with service users in the community to secure permanent housing and will provide ongoing case management supports for up to 24 months
  • Will partner with various agencies to leverage and coordinate the assets and services available in Hamilton to ensure that service users have their individualized housing and support needs met
  • Work takes place in the office, overflow hotels, emergency shelters, and in the community
  • Accompany service users to community appointments, apartment viewings, and will attend service users’ homes for monthly home visits
  • Must be willing and able to assist service users in a hands-on capacity
  • Time management skills are necessary in a high energy, changing environment
  • Ability to work independently

 

Qualifications

  • Minimum 1 year related experience or equivalent combination of education, training and experience in Social Services or related field
  • The successful candidates will have a demonstrated knowledge of Housing First frameworks and the ability to assist service users with system navigation A post-secondary education in a social service related field is required
  • Fluency in a 2nd language is an asset
  • Proficiency in Microsoft Office programs and strong written and oral communication skills required
  • CPR certification or willingness to be certified is required
  • Valid G license required
  • An in depth understanding of trauma, addictions, mental health, and homelessness is required
  • Creative problem solving and advocacy skills required
  • Hours of Work: Shifts are day shifts (9 am to 5 pm), one evening shift per week (11am to 7 pm) and one Saturday approximately every six weeks (9am to 5pm)
