TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking a Transitional Care Case Worker to join our team in a full-time permanent role. TNO is a great place to work and we make a difference in the communities we serve.
A Bachelor’s of Social Work (BSW) is required for this role, and the ideal candidate will have 2-3 years of experience in crisis and trauma support. Additional training, education or certification in the areas of crisis or trauma management is an asset. Registration with the Ontario College of Social Workers and Social Service Workers is also required. Proficiency with MS Office applications and standard office equipment, as well as the ability to design flyers and presentations is necessary.
As a Transitional Care Case Worker at TNO, your main responsibilities will include engaging with clients and creating personalized outreach plans for each assigned service location in collaboration with other TNO programs or external service providers. You will facilitate connections between program participants and essential services such as case management, one-on-one mental health support, and rehabilitation services. Additionally, you may need to conduct home visits to actively support clients throughout their pre and post-treatment journey. Ensuring accurate maintenance of program records and timely submission of weekly and monthly reports will be crucial. Lastly, you will need to effectively coordinate activities with other TNO departments and services in order to provide comprehensive support to clients.
Vaccination Requirement – You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.
Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Schedule: Monday to Friday, 9 a.m. to 5 p.m. – Some evenings and weekends may be required
