TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking a Social Worker, Mental Health to join our team in a full-time permanent role for our Integrated Mental Health Support Services program. This is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by OH-Toronto. This new community mental health initiative is aimed to enhance integrated mental health services to enable appropriate, coordinated and effective health service delivery in the Flemingdon Park, Thorncliffe Park neighbourhoods, and surrounding areas.

A Master's Degree in Social Work, and registration with the Ontario College of Social Workers is necessary for this role. In addition, a minimum of 3 years of social work experience in a healthcare setting or social service agency is required. One of those years can be a social work internship. Preference will be given to candidates with 2 years of recent post-master’s experience in a clinical setting. The ideal candidate will have excellent assessment, interviewing, and counseling skills, with expertise in mental health and substance abuse. Knowledge of psychopathology and therapeutic interventions, including psychopharmacotherapy, is desirable.

In this role, you will provide psychosocial assessments, diagnoses, and treatment for patients. Additionally, you will assist patients and their families to manage various aspects of their situation, including hospitalization, illness, diagnosis, treatment, and emotional or mental challenges. You will need to ensure that patients receive care in compliance with various policies, procedures, standards, and regulations. To do so, you will employ excellent decision-making skills and know when to refer patients for further medical assessment. You will collaborate with patients, families, and the healthcare team to plan and consult on patient discharge. The role also requires sensitivity, tact, diplomacy, and professionalism in all interactions.

Vaccination Requirement – You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type: Full-time permanent position

Schedule: Monday to Friday, 9 a.m. to 5 p.m. – Some evenings and weekends may be required
