Annual Salary: $56,305.60-$67,620.80

Founded in 1960, the Youth Services Bureau of Ottawa (YSB) is one of the largest and most comprehensive non-profit agencies in Ottawa and a registered charity. It provides mental health services, employment programs, emergency shelter, community, health and housing services, and justice programs to youth in the Ottawa area, serving between 2,500 and 3,000 at‑risk individuals aged 12+ and their families every month. YSB employs 350 caring professionals in a unionized environment in over 26 locations across Ottawa.
Under the direct supervision of the Assistant Director of Programs and Services, Case Managers work as part of a multidisciplinary team at Youth Justice and are the identified team members directly responsible for establishing, implementing and facilitating a comprehensive case management plan for each youth engaged with Youth Justice Services from admission to discharge, adhering to agency policies and procedures.
What you will do:
- Provide ongoing case management services to clients to support youth from admission to discharge.
- Maintain current client files, complete case management reports and documentation (including goals/action), and summary reports.
- Build relationships and rapports with clients, families, and community partners.
- Complete assessments, identify strengths and needs, and complete appropriate referrals.
- Connect youth to community supports, links to employment and/or education opportunities and support youth in obtaining long term housing as needed.
- Participate and lead programming and life skills.
- Comply with all mandating legislative requirements, Agency policies and procedures.
- Safeguard client confidentiality, rights and responsibilities.
- Advocate for clients and facilitate access to community resources and services.
- Represent the Agency in a professional manner. Maintain positive working relationships with the community, other services providers, and collaterals.
- Attend and participate in supervision.
- Participate in on-going supervision of part-time/students.
- Follow safe work practices as established by the Bureau and the program.

- Ensure compliance with Agency Policies and Procedures, Program Policies, Collective Agreement & relevant legislation
This opportunity may be right for you if you have the following:
- A diploma or degree in the social sciences
- A minimum of two years of case management experience
- Proven familiarity of community services and internal/external referrals
- Proven written & verbal communication skills
- Ability to work autonomously and as an effective team member
- Willingness to work up to two evenings per week, and occasional weekends
- Fluency in English and French
