We are looking for a Registered Practical Nurse on our Assertive Community Treatment Team

As we continue to grow our team, we are looking for a Registered Practical Nurse to join our passionate, dedicated, and "BRAVE" team! Contract: 18 month, full-time, position available. 35 hours per week. Some evening and weekends hours may be required

Location: In accordance with current public health directives, training will be available both remotely (i.e., via teleconference and videoconferencing technologies) as well as onsite while respecting all public health protocols and physical distancing . During the current COVID-19 outbreak, work can be completed from home with equipment provided by CMHA. Once public health directives are lifted, the Registered Practical Nurse will be working primarily in the community and our Alliston Office (35 Victoria St E, Unit 1, Alliston, ON  L9R 1T3) and will be required to travel within York Region & South Simcoe.

Job Profile:

The Registered Practical Nurse (RPN) works with clients 16 years of age and older who are experiencing significant challenges with activities of daily living, and functioning across all domains – health, educational, vocational, emotional, social, recreational, and financial as a result of serious and persistent mental illness. In collaboration with the ACT team, the RPN conducts mental and physical health assessments, develops plans, coordinates and provides treatment and rehabilitation, as well as professional support. The RPN makes referrals to community resources, including community physicians, and coordinates consultations from other health professionals. Using an Empowerment/Recovery approach, the RPN provides support, assistance, counselling, referrals, and education to clients, and with their permission, their families.

The RPN is responsible for the management and administration of medication within their scope of practice and in consultation with the attending ACTT psychiatrist. The RPN works collaboratively with other team members, sharing shift management responsibilities. They may be assigned to different programs as required based on need. (i.e. Case Management, etc.).

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building in a collaborative environment and demonstrate confidence in decision-making and in building relationships.

Requirements:

  • A satisfactory Vulnerable Sector Screening (Police Check)
  • A valid Canadian driver’s license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
  • Ability to apply Recovery principles and empowerment oriented philosophies and practices in work with clients
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
  • Experience with administration, injection, side effects and iatrogenic manifestations of psychotropic medications, especially the newer neuroleptics
  • Experience working with individuals experiencing serious mental illness, concurrent disorders and dual diagnosis (developmental disability and/or traumatic brain injury) is preferred
  • A second language (French, Cantonese, Mandarin, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu, French) reflecting the local community is preferred
  • Excellent health assessment skills, sound clinical judgment and diagnostic skills
  • Four or more years’ experience as an RPN, preferably in a mental health setting
  • Member in good standing with the Ontario College of Nurses
  • Licensed Registered Nurse (B. Sc. N degree) or Registered Practical Nurse (Dipl. N.)
  • Experience working as a case management in mental health and addictions.
