Founded in 1965, Christian Horizons is a non-profit, faith-based charitable organization supporting people with disabilities in Canada and around the world. With over 3,500 employees, Christian Horizons supports nearly 2,000 people with developmental disabilities and their families. Christian Horizons supports people within their communities throughout Ontario and Saskatchewan. Internationally, Christian Horizons works in partnership with local organizations to promote the inclusion and rights of people with disabilities in six developing countries.

FULL-TIME HOST FAMILY COORDINATOR

(35 hours/week)

Waterloo-Wellington Host Family Services –“LifeShare”

Working throughout the Waterloo and Wellington regions, the Coordinator will be required to travel a part of this role, and work flexible hours, including, evenings, weekends and on-call. The Coordinator is responsible to promote, recruit, coordinate, monitor and maintain Host Family residential and respite services within their designated geographical or case management assignments. The Coordinator is responsible to facilitate and support matches between Homesharers (adults/children) and approved Host Families, seeking to promote Christian Horizons’ Service Principles in the lives of people supported, and ensure service provision follows Christian Horizons’ policies and procedures, Ministry of Children and Community Social Service legislation, and Ministry Host Family Services Directives.  You will work in collaboration with relevant community partners in support of the needs and goals of Homesharers and Host Families. You will be required to respect and uphold the Values, Vision, and Mission of Christian Horizons, demonstrating the highest standards of courtesy and respect in their dealings with other employees, persons supported and both internal and external stakeholders.

 

Requirements: College Diploma or University Degree in a related field; Developmental Services and Social Work education preferred, as well as 3 years of work experience. First Aid/CPR and Non-Violent Crisis Intervention Certification are preferred assets. Strong assessment, screening, case management, along with problem solving and conflict resolution skills are valuable assets. Exceptional interpersonal and business communication skills, including some public speaking, report-writing, and computer proficiency, are required. The successful candidate will demonstrate the ability to work independently and flexibly within a team environment and with community processes and service partners. Established relationships and networks within the regions of Waterloo and Wellington is an asset, as is the commitment to a person-centered model of support. Involves working with people who may be under significant personal stress or who may have challenging behaviours; intermittent periods of considerable mental demands. A valid Ontario G Class Driver's License and access to reliable transportation is required. The Coordinator will be expected to work flexibly; including evenings and some weekends, and on-call.
