Social Prescribing Manager

The successful candidate will be responsible for training and overseeing employees and volunteers who are working together to link seniors with resources and supports and will help to facilitate more complex Social Work intervention cases. They will facilitate supporting the health and well-being of seniors by working with other program partners. They will help to develop program policies and procedures and evaluation.

Qualifications of this position include:

  • Social Work degree or diploma
  • Registered Social Worker
  • Undergo Police Information Check including Vulnerable Sector
  • Access to a reliable vehicle and a valid Class 5 driver’s license;
  • Be able to obtain $2 million auto liability insurance
  • Have a clear understanding of inclusive, anti-oppressive practice
  • Employees are required to be fully vaccinated against Covid-19

The position is a permanent full-time (35 hours/week) job opportunity. $47,754 base rate per annum, plus Employer paid Health & Dental plan and a matched Pension plan. Sage offers an outstanding work life balance beginning with 3 weeks of paid vacation plus the last week of December off with pay, and generous sick leave accruals. This position also qualifies for a compressed work week if desired. Rates above the base rate will be considered based on assessment of the successful candidate’s education, skills, previous experience and other factors. Sage is offering a hybrid working environment at this time.
