Position:                     .5 FTE Part Time Peer Social Recreation Assistant

 

Purpose:                    This is a non-clinical peer role, reporting to the Clinical Manager, Case Management & Health and Wellness Programs. The Peer Social Recreation Assistant provides support and transportation for the Health and Wellness Program group activities and outings, and to large agency wide events for persons engaging in services. This position promotes wellness and recovery through community involvement and group participation, and endeavors to meet clients’ physical, social, emotional, intellectual and cultural needs and interests.

 

Key Requirements:

Program Support

  • Under the supervision of the Clinical Manager, Case Management & Health and Wellness Programs, provides assistance to the Social Recreation Worker, to develop and deliver a variety of social recreational activities and groups.
  • Escorts and supports client groups to special events and community outings.
  • Participates in and supports a Health and Wellness Program member advisory committee.
  • In consultation with the Social Recreation Worker, ensures that all equipment and supplies are available and in working order, for social recreational activities.
  • Supports the evaluation of the program, through distribution and collection of client feedback tools, on an ongoing and annual basis.

 

Transportation

  • Transports clients to on site agency Health and Wellness programming as well as scheduled outside activities or events such as community events, special outings and group activities.
  • Works collaboratively with the Social Recreation Worker and Coordinator of Volunteers, to confirm the transportation scheduling of individuals in advance.
  • Works collaboratively with the Social Recreation Worker and Coordinator of Volunteers to respond to other special driving requests.
  • Supports passengers to follow guidelines for passengers while enroute in the agency vans.
  • Reports any issues or concerns to the Clinical Manager, Case Management & Health and Wellness Programs.
  • Develops the most efficient route to follow when transporting passengers.
  • Records mileage and destination on the SHOP Van Log Sheet on each use and submit the completed van log to Clinical Manager, Case Management & Health and Wellness Programs at the end of each month.
  • Ensures clean condition of van and remove all garbage from van.
  • Ensures van is in safe working condition and complete Van Daily Circle Inspections- pre-trip/post-trip and Van Use Log.
  • Takes agency vehicles in for regular maintenance and obtains necessary supplies for van (e.g. fuel, windshield washer fluid, oil, etc.).
  • Reports any van damage or safety concerns to the Clinical Manager, Case Management & Health and Wellness Programs.
  • Follows all SHOP Passenger Van and Safety Policies.
  • In bad weather, confirms with Clinical Manager, Case Management & Health and Wellness Programs if transportation will or will not operate on that particular day.
  • Complies with all responsibilities in the Occupational Health and Safety Act, Ministry of Transportation, Highway Traffic Act, all SHOP Policies and Procedures and agree to treat health and safety with a high priority while employed at SHOP.

 

Peer Support

  • Initiates, establishes and maintains relationship with peers while developing trust and rapport.
  • Acts as a role model in recovery.
  • Shares common experiences with peers as appropriate.
  • Works collaboratively with peers, co-workers and the community.
  • Supports peer to address problems/issues and find answers.
  • Supports the peer to gather information related to self-recovery.
  • Supports peer to set goals and work toward developing coping skills.
  • Supports peer to create a life of their choosing in the community.
  • Promotes social recreation as a recovery tool option.

 

Administrative

  • Documents in agency electronic client record.
  • Maintains up to date records regarding client activity attendance and use of services.
  • Contributes to and supports the distribution of the monthly calendar and newsletter for the Health and Wellness Program.
  • Maintains equipment and work areas.
  • Ensures the safety and security of participants and maintains a clean, tidy program environment.
  • Distributes, collects and records client survey in accordance to funder requirements.  
  • Makes suggestions for program and quality improvement to Clinical Manager, Case Management & Health and Wellness Programs

 

Shifts:                                  Will take place between 8:30 a.m. - 4:30 p.m. with flexibility as required by program needs, 20 hours per week

 

Main Location:                    Milton and Burlington with travel to Halton Hills and Oakville

 

Duration:                              September 2019 to December 2022
