Our Programs team provides a holistic model of care for Indigenous community members and seeks a positive and experienced manager to support day to day operations of the diabetes and health promotion team. This includes leading a team of interdisciplinary staff in coordinating culturally-based diabetes management and health promotion service and activities for Indigenous community members in Ottawa.

Purpose of Position:

Reporting to the Director of Programs, the Diabetes and Health Promotion Team Lead will guide the team in their roles of culturally-based diabetes management services and health promotion programming.  They will be responsible for meeting service requirements outlined by funders, quality assurance, community and stakeholder relations, staff development, and supporting the Director to operationalize strategic directives.

 

Responsibilities of this position are:

  • Lead and support team members through completing team and individual work plans, meeting goals, reporting, problem solving, and performance management;
  • Source consultants (ex. Elders, traditional educators, facilitators), manage contracting of consultants for services, events planning and program delivery;
  • Recruitment and onboarding of staff and consultants;
  • Support team to provide diabetes management in accordance with clinical practice guidelines, promote collaboration with the primary care team, systems navigation, and manage referrals from Indigenous and mainstream organizations;
  • Build and maintain relationships with community partners;
  • Conduct program evaluation, monitor data collection, and reporting.

 

Qualifications:

  • Bachelor Degree in Health Promotion, Social Work, Health Administration; Masters Degree will be considered a strong asset.
  • Minimum three years’ clinical supervisory experience managing a multidisciplinary team in health care or social services.
  • Strong knowledge of chronic disease, diabetes management and health promotion concepts;
  • Knowledge of Indigenous peoples’ history and challenges Indigenous peoples face in contemporary society and within the healthcare system;
  • Demonstrated leadership experience in addressing issues of inequity, racism, and oppression on an individual, organizational, and system levels.
  • Demonstrated leadership experience in partnership development;
  • Excellent problem solving ability with sound judgment, strong oral and written communication skills;
  • Ability to work effectively in a fast-paced environment;
  • Knowledge of available resources in Ottawa and ability to research the resources available within the broader diabetes management and health promotion field;
  • Strong promoter and enhancer of team work;
  • Proficient in Microsoft Office Suite;
  • Experience with Electronic Medical Records systems;
  • Strong organizational skills and the ability to handle sensitive issues with tact and diplomacy;
  • Ability to work a flexible work week that may include non-traditional hours, evenings and weekends.
