The North York Toronto Health Partners (North York’s OHT) is working in shared purpose across our partnership to improve healthcare for the NYTHP OHT attributed population and is looking for a Coordinator to work with the partners of this OHT. One of the key responsibilities will be to facilitate case resolution for clients with complex mental health and addictions issues.

Service resolution tables are committees that bring together high level managers and decision-makers from a cross-section of organizations from multiple health social service sectors. The table’s purpose is to engage in creative and collaborative problem solving to provide customized service solutions for individuals with mental health issues, addictions, and/or dual diagnosis who have continually experienced barriers in accessing services and getting their needs met. Working with partners of the OHT and additional stakeholders, the Coordinator is responsible for developing materials needed to support this new initiative, coordinating case resolution meetings, and the compilation of data and lessons learned through service resolution to inform system change. 

In addition, the Coordinator will be part of a foundational committee of the NYTHP (Backbone Committee) to support the OHTs mental health and addictions initiatives and build capacity to meet the needs of NYTHPs attributed population.

The Coordinator position will be hosted by Toronto North Support Services (TNSS) on behalf of the NYTHP OHT. As such, although the position will have a reporting structure within TNSS, the successful candidate is fulfilling a NYTHP OHT role and will be responsible for the duties set out by the NYTHP OHT.

RESPONSIBILITIES AND DUTIES

1. Service Coordination

Provide service coordination and facilitate case resolution for individuals with Mental Health, Addictions and/or Dual Diagnosis through the following skills and activities:

  • Provide advanced sector specific knowledge – Mental Health and Addictions, Dual Diagnosis, Collaborative Tables
  • Establish streamed referral pathways, relationships and processes for partner agencies and ad-hoc partners
  • Refer and liaise with other service providers as required
  • Provide advocacy, crisis prevention and intervention, for complex cases
  • Assist service providers to develop and identify crisis management plans on behalf of clients
  • Assist service providers to identify goal areas and specific objectives on behalf of clients within the scope of case resolution
  • Provide support, education, and information to partnering agencies
  • Identify problem-solving strategies for resolution of immediate needs
  • Liaise with cross-sectoral referring pathways, and other service providers to coordinate supports for service resolution
  • Facilitate local case resolution through active consultation and participation in collaborative system efforts where the focus is to address complex or critical service user needs and supporting individuals who require a multi-service approach
  • Work with the governance structure of the Service Resolution Table to meet the needs of individuals who are experiencing complex, mental health and addictions issues and/or Dual Diagnosis
  • Participate in evaluation of partner, client satisfaction
  • Use available technology to locate supports and resources, and coordinate, and facilitate Service Resolution Table meetings
  • Gather data and identify service gaps where there are community development needs and system improvements required
  • Work with service resolution partners to escalate system gaps to the Mental Health and Addictions Steering Committee

2. Support for NYTHP Mental Health and Addictions (MHA) Initiatives

Support NYTHP members from across the partnership to drive the work on MHA initiatives through the following skills and activities:

  • Coordinate partners and external stakeholders to develop and implement OHT MHA initiatives as needed
  • Work closely with the current MHA access points to further integrate the access points, the services provides, and address gaps
  • Facilitate and support the planning of select OHT MHA focused working group meetings
  • Conduct background research to support OHT MHA focused working group meetings, as required
  • Prepare agendas, minutes and invitations for MHA focused working groups and project touch-bases
  • Participate on NYTHP committees, as required

Qualifications:

  • Master in Health and Human Services or equivalent experience and a minimum of 5 years direct clinical experience in the area of complex mental health and addictions
  • Demonstrated experience coordinating and supporting projects
  • Excellent planning, organization, problem-solving, decision-making and liaison skills, and leadership skills
  • Excellent knowledge and understanding of mental health and addictions community resources
  • Excellent verbal and written communication skills
  • Ability to collect and communicate data to various stakeholders
  • Strong relationship management and community engagement skills

Working at Toronto North Support Services means working with a dynamic team of fellow healthcare providers, staff, and volunteers.  This is a Temporary Full Time Regular position, 70 hours bi-weekly, daytime hours, 8 hour shifts.
